Creating a Health Home To Serve Homeless Patients

There are a number of initiatives currently underway in California that will have a profound effect on the service delivery system and responses to homelessness. The Affordable Care Act's (ACA) Health Home Program for Medicaid Beneficiaries with Two or More Chronic Conditions (Section 2703) provides an opportunity for states to offer an optional benefit to fund services that coordinate care across medical, mental health, substance use, and social services systems. California is preparing for implementation of a Health Home Program.

Join us for this 60-minute webinar which is a follow-up to our first session on April 26, "Opportunities to Address the Needs of Your Homeless Patients". Corporation for Supportive Housing (CSH) will present a deeper understanding on the impact of California's Health Home Program (HHP), and how to address the needs of homeless and frequent hospital user beneficiaries through the health home benefit in California.

  • Description of the 10th Decile Project and outcomes achieved
  • The role of health centers and FQHCs in addressing the health needs of homeless people
  • Partnerships and behavioral health integration, including potential partnerships under the Health Home benefit
  • Staffing models under current programs and the Health Home benefit
  • Working with managed care plans to become a Community-Based Care Management Entity and receive the homelessness modifier through the Health Home Program
